Clincial Study on Abruptio Placentae

Abstract
We investigated 28 cases of abruptio placentae among 9,502 deliveries in the Department of Obstetrics and Gynecology, Seoul Paik Hospital from March, 1983 to Februray, 1993.
@ES The results were as follows :
@EN 1. The incidence of abruptio placentae was 0.29% or one in 339 deliveries.
2. The age distribution at the time of diagnosis revealed that 14 cases (50%) were from 26 to 30 years, 9 cases (32.1%) were from 21 to 25 years, 5cases (17.9%) were from 31 to 35 years, 9 cases (32.1%) were from 21 to 25 years and 5 cases
(17.9%)
were
from 31 to 35years.
3. The incidence of abruptio placentae in nulliparous woman was 32.1% and in multiparous woman was 67.9%.
4. The incidence of unknown etiology was 12 cases (42.9%) and pregnaccy induced hypertension was the most frequent etiologic factor, comprising 35.37% of cases.
5. Uterine tenderness and/or back pain was the most common clinical symptom (78.6%) and vaginal bleeding was found in 19 cases (67.9%), fetal distress was 17 (60.7%), intrauterine fetal death was 8 cases (28.6%), uterine hypertonus was 7 cases
(25%),
and pretem labor 5 cases (17.9%) respectively.
6. As for the distribution of birth weight, 16 cases (53.3%) were less than 2,000 grams and 10 cases (33.3%) were 1,500 to 2,000 grams.
7. Concerning the correlation with gestational Age, neonatal condition the shorter gestational age, the worse neonatal condition.
8. In 18 cases (64.3%), blood trasfusion was necessary.
9. Vaginal delivery was performed in 75.0%, cesarean section in 25.0%.
10. The complications of abruptio placentae in our studies were coagulopathy in 10.8%, hemorrhage & shock in 7.1% and renal failure in 7.1%.
11. Perinatal mortality rate was 32.2% including still birth (28.6%) and neonatal death (3.6%).
